Meal Payment Information

Please be sure there is money in your child’s account. When sending checks, please be sure to put your child’s name on the check. If sending cash, please put it in an envelope with your child’s name. YOU MAY SEND ONE CHECK FOR ALL YOUR CHILDREN IF YOU CHOOSE. You may pay by the day, week, month, semester or year.

The completed AD-3027 form or letter must be submitted to USDA by:
