All meals, foods and beverages are prepared and served by qualified child nutrition professionals. We provide students with access to a variety of affordable and appealing foods that meet the health and nutrition needs of students.

Does your child have a food allergy, food intolerance, or meal accommodation need? LUSD Nutrition Services cares about providing food to
all students. With the help from our School Nurses and Nutrition Specialists, our Nutrition Services department takes pride in providing necessary accommodations and choices for students. 

How do I start my child on a special diet?

Nutrition Services is happy to work with students who need meal substitutions on a case-by-case basis. Parent/guardian requests must be supported by a written medical statement from a state licensed healthcare professional. Medical Statements can be retrieved directly from School Cafeteria Managers, School Nurses, the Nutrition Services office, or downloaded right here.

How do I fill out a Medical Statement form? What do I do with it after it is filled out?

Parents/guardians can use the following example to see a properly filled out form:

Once filled out by a parent/guardian, the form must be approved and signed by a state licensed healthcare professional. The CDE only permits the following state licensed healthcare professionals to complete and sign a written medical statement for a disability: licensed physicians, physician assistants, or nurse practitioners.

After the form has been returned and signed by a state licensed healthcare professional, the parent/guardian can submit the form to the school site Cafeteria Manager, school office, or mailing it to Nutrition Services.

What if my child does not have a disability but I still want to request a diet accommodation due to religious, philosophical, or ethical reasons?

Nutrition Services offers a variety of meal choices, including vegetarian options, in order to accommodate student preferences. At this time the CDE only honors non-disability requests for fluid milk substitutions.
