If your student needs a meal modification or has a food allergy that we need to replace please fill out Special Diet form and return it to your students school. Students who need their school meals modified to accommodate food allergies or preferences will be required to submit a Meal Modification Form signed by their medical provider. The Food Services Department will only modify meals if this form has been completed by a medical professional.
